These forms are required to be filled out prior to students participating
on any of the athletic teams at Homer Central School District.
Guidelines for Athletic Behavior
Parental Medical
Consent Form
- Must be signed by the parent.
Physical Exam Form - Must
be filled out and signed by a physician.
Transportation Release
Form - Must be signed by a parent and an authorized
administrator.
Interval Health History Form - Must
be filled out and signed by parent/guardian and either school nurse or
physician. This form must be filled out within the 30 days prior to the first practice. It can not be accepted if filled out prior to the thirty days.
