NOTE: Students entering sixth grade on or after September 2007 must be immunized with the Tdap vaccination. This is a new State Law. If your child is ten years old upon entering sixth grade then they must be immunized once they turn eleven. Students not complying with this law will be excluded from school. Please contact your medical provider for further information regarding this new State Law.

NOTE: Effective Jan. 1, 2005 - All students entering sixth grade need to either be immunized against varicella (chicken pox) or have proof of having had the disease. Immunization records are required. The only acceptable proof of having the disease is a physician document. (Parent memory or recall will not be accepted.)
