New York State law requires that each student participating in sports has a current physical on file in the nurse’s office each year to participate in sports.
One physical exam per school year is sufficient for all sports that school year.
A physical exam is considered current for twelve months from the date acquired. If the student has a current physical performed by their treating medical provider a school sports physical is not required to participate.
The school arranges for sports physical exams prior to the start of each season.
If a student does not receive a school sports physical, they must submit a current physical from their doctor to participate.
Fall sports physicals will take place at the Homer High School Nurses office on the following dates:
Thursday, July 27, 2017 1:00-4:00 PM
Tuesday, August 1, 2017 8:30 AM-12:00 PM
Thursday, August 3, 2017 5:00-7:00 PM
Additional health forms may be required:
- Completed Pre-Participation Physical Evaluation for those students receiving a school sports physical.
- Completed Interval Health History for those students who have received a medical examination 30 days prior to the start of the season.
Blue Cards will be provided when all the required documents are received.
Jr. High Blue Cards will be issued at the Jr. High Nurses office for fall sports on:
August 30 and August 31st. 8:30 am-2:30 pm
High School Blue Cards will be issued to the coaches on Thursday, August 10th for those athletes that have been cleared. The high school nurse will be in the office from 9:00 AM – 2:00 PM on August 10th to collect any additional paperwork listed above that students are missing.
The nurse can be reached at 749-1211 on August 10th. If athletes do not have their blue card on the first day of practice, 8/14 or 8/17, they may go to the Homer Elementary School nurse's office to pick them up.
